To request a court or field, the DPW Permits Office requires that all applicants submit a certificate of insurance, game and practice schedule, and roster, in addition to submitting the online application that can be accessed at https://forms.office.com/g/csZYuf2iGW . If you are unable to submit a request via the form, phone or in-person reservations will be done by appointment only. Please email dpwfields&parkshelters@pittsburghpa.gov to schedule if you are unable to use the DPW Permits Office’s online service.
